These terms and conditions, authorization and consent for our products or services are signed in between: Patient, his representative/relative, any other decision maker, patient care facility or any individual who has expressed interest in our products or services here in Called as Client/Customer and www.Jeevanjyoti.in as facilitator of services and products mentioned on its website.

AGREE TO BE BOUND BY THE TERMS AND CONDITIONS, MEDICAL EQUIPMENT ON RENT, AUTHORIZATION AND CONSENT SET OUT BELOW.
Medical equipment on rent:
Medical equipment given on rent are property of jeevanjyoti.in they are given on rent and not sold. No government taxes for the purpose of sale have been collected. Security deposit taken against the equipments is refundable after deducting the mentioned outstanding rent. In a case where the security amount becomes equal to the out standing rent; It will not amount to understanding that the equipments are now owned by the client/customer/lessee. It will become the responsibility of the client/customer/lessee to return the equipments and settle the account OR renew by paying a fresh security deposit.
We advice if you require the equipments for a longer term, you should buy them from the open market, pay the government taxes and return our equipments and settle the account.
Medical equipment rented by jeevanjyoti.in are not manufactured by jeevanjyoti.in. All the warranties /guarantees etc are of the respective manufacturers. By placing an order client/customer/lessee agrees not hold jeevanjyoti.in responsible for any losses or loss of life owing to malfunction or defect arising in any equipment. In such event we will try our best to rush the replacement but we do not guarantee the replacement. It will remain responsibility of client/customer/lessee to have alternative arrangement ready in advance.
Authorization and Consent
I here by authorize jeevanjyoti.in and whomsoever they may designate to administer the required services of Ayah, attendant, nurse physiotherapist etc as the case maybe to perform the procedures as assigned to them. And such additional therapies/intervention/procedures as are considered therapeutically or diagnostically necessary during the course of procedure/treatment. I also authorize Ayah, attendant, nurse physiotherapist etc to perform the procedures at my home or in any alternative medical setup as deemed appropriate by them. I hereby also grant permission and consent for the storing of my medical data in the format of secured electronic media records. Aggregate and thoroughly anonymized version of this data may be later shared with third parties.
I have been explained that during my time as a client of jeevanjyoti.in, I may be administered medication orally, intravenously, by IV infusions or by any other route as required. I have been adequately explained that the risk of any administering of the above could be minor/major/severe reaction with rare serious/fatal consequences.
I have also been explained that facilities better than those offered by jeevanjyoti.in may be available at other places to deal with emergencies arising out of these situations or arising out of the disease itself.
I do understand and appreciate that particular guarantee or assurance cannot be made as to the outcome of procedure or treatment that I may receive. I also certify that no guarantee or assurance has been made as to the result that may be obtained. I am fully aware that any treatment/procedure, including the ones that I have to undergo may have a risk of failure, and/or additionally a risk of limiting my functions, or of incapacitating me. I am also aware that, very rarely, other unforeseen risks or complications not discussed may occur.
I understand that the Ayah, attendant, nurse physiotherapist etc assigned to my care are not the employees of jeevanjyoti.in. They are freelancers who have been verified for their background qualification etc to perform the assigned job. Under no circumstances I will hold jeevanjyoti.in responsible for the misdeed or misconduct by any Ayah, attendant, nurse physiotherapist etc facilitated by them for my care.
Any non-patient care related works like personal work of other family members, fetching medicines, grocery etc from out side, kitchen work, moping, washing utensils etc. All of which constitute non-medical non patient work will not be given to them. Also I will not ask anything that is not discussed in treatment plan laid by co-coordinating/medical team OR that is not mention in their respective profiles below:
Attending to the comfort of women of all ages.
Attending to the comfort of mother with new born infant for post delivery recovery.
Helping mother with infant-related tasks.
Her primary charge is childcare and childcare assistance to a parent plus baby-sitting.
She may or may not be willing to do other light housework but their priorities will always be in the care of the children. She will report directly to the lady of the house.
His/her duties are classified as routine tasks involving no risk for the patient. Provide basic patient care under direction of nursing staff. Perform duties such as feed, bathe, dress, groom, or move patients, change the linens, diapers and dispose them. May transfer or transport patients. Will not administer medicine to patient but can only give medicine if given to give by the relative of the patient.
Their respective jobs as described in their 'job description' as a “qualified nurse”.

Duty hours for ayah, nanny, attendant, nurse shall be 8am to 8pm shift, 8pm to 8am shift Or 24hrs for the required number of days.
I also undertake that I am not withholding any relevant information that my ayah, attendant, nurse, physiotherapist etc may need for the proper conduct of their respective duties. I have been given the opportunity to ask any questions that I wanted to ask and I have received answers to my satisfaction. I will not ask or request my ayah, attendant, nurse, physiotherapist etc to stay beyond their duty hours. In case owing to any circumstances, if my aide has to stay beyond his/her duty hours; I will be liable to pay by “per hour basis” for additional each hour @ 20% of the basic 12 hour duty rate.
In case of ayah, attendant an advance deposit amount of 3 shifts and for nurse an advance deposit amount of 7shifts is mandatory. In case services are used for lesser shift or terminated, the amount for the remaining shifts shall be refunded.
Services and products are subject to availability with us or our vendors. In case of non availability you may choose to
1. Accept a partial / full refund or take it as a credit in your jeevanjyoti account.
2. Refunds can be made to the same account only.
3. In case of price difference the amount can be refunded to you or can be issued as account credit.
Ensure you enter a correct email address and we would send you an order acknowledgement email and order update email(s).
Unless we have notified you that we do not accept your order or you have cancelled it, order acceptance and the creation of the contract between you and us will take place at the point the services or products you have ordered are sent from our facility to be reached or delivered to the address you have given us. It does not take place until that stage, even though we may have debited your card (see 3 above) or we have sent acknowledgement emails.
The contract will be formed at the place of dispatch of the products.
All orders that you place on this website will be subject to acceptance in accordance with above of terms and conditions.

Shipping inside India is free of cost. We use reputed couriers like Blue Dart, First Flight, DTDC etc to ship the products.
If your order has multiple products with different dispatch dates, we would have all the products shipped as one shipment, once all products in your order is ready. However, if you wish to receive some products earlier, please email us, We shall do the needful.
Couriers may call you before delivery. In case you are not available during delivery, you could call them back and arrange for re-delivery or you can pick it up from their location. We are not responsible for any delay caused by any unforeseen time lags by the courier companies in delivering the parcels to the customer, or if the customer is not available at the time of delivery.

HOW TO SEND RETURNS
Returns would not be accepted for, opened, broken, damaged or tempered products. However, we would accept returns In case of genuine fault from our side. At our discretion we may replace or exchange the product or offer a coupon, excluding the shipping cost. Additional charges if any would be applicable extra.
We do not accept returns for any product purchased during clearance sale/ discounts etc.
We do not accept any returns after 4 days of receiving the shipment.
All products should be returned in original condition with tags intact as attached. Presence of original tags is a must to accept returns.
Any complaint with regards to damage or stains or any other should be reported to us within 24 hrs of the receipt of the products. We may ask for a digital picture of the same in order to review the same.
Refund on Returns:
We will not refund the custom duties, taxes, if applicable, or paid by you at time of receiving the goods.
At any point of time the refunds will not exceed the actual value of the goods.
There is an admin/credit card commission cost of Rs 200 will be deducted for refunds. If refund is issued as a gift Coupon there is no charge applicable.
For orders placed using Bank Transfer payment method refunds would be issued only as Gift Coupons and in such cases there would not be any admin cost as mentioned above.
Shipping cost would be deducted for refund, since we offer free shipping all over.

SHIPPING & DELIVERY
We ship to any Indian address we do not ship or deliver to PO Boxes.
A shipping charge is applicable to every dispatch. Sometimes Shipping charges are fully or partially waived for shipping based on the promotions.
All delivery deadlines will depend on address details supplied and stocks being present at time of dispatch. If there are any discrepancies in your order we will contact you, so please be sure to provide correct contact numbers with area codes, as well as an email address.
We will try to deliver all orders received, however if you are not at home, in case of service a card will be left in case of a product the parcel will be delivered to a neighbor, or returned back to the depot. It is your responsibility to collect it or arrange re-delivery at your cost.
If a parcel is returned as undelivered we will offer a refund for the items only. If you still require the parcel to be resent a further shipping charge is payable by you. This includes any shipping charge that was paid by us for receiving the parcel back.
In some cases, if you are not reachable unavailable at the given address for more than 4 days, the courier company would destroy the parcel. In such cases, we would not be responsible for the parcel. No refund claims would be entertained in such scenarios.
We shall have no liability to you in respect of any delay or failure to deliver a product due to circumstances beyond our reasonable control, including (without limitation) delay or failure caused by adverse weather, strikes, custom clearances, transport problems, for delays caused by acts or omissions of your details or other regulatory agencies.
We are not liable to pay refund for late delivery if the consignment does not have a complete and accurate address, post code and telephone number for the customer.

Refunds would be processed only if the product tags are intact and is in the same condition in which it was delivered to you.
